There are various positions within the U.S. government that are appointed rather than elected. Some of the titles belonging to appointed positions are the Postmaster General, the Attorney General and the Secretary of State.
Supreme court justices are not elected by US citizens at large. They are nominated (not appointed-- if they were appointed then they could just take office) by the president, and then the nominee is either confirmed by the Senate or denied. If a presidential nominee is denied, the president makes another nomination. Theoretically this just keeps on until a nominee is confirmed. The president is not technically elected by the citizens, but he is not appointed either. He is elected by the electoral college. A vice president becomes president if the president is not able to continue in office, and then we have a president who was neither directly elected nor appointed. But all of this is part of what the founders built in to guarantee orderly transfer of authority.
Subject to Congressional approval, the U.S. President appoints the Vice President when there is a mid-term vacancy in the office, Supreme Court Justices, Cabinet Secretaries, Ambassadors/ Foreign Ministers, Army Generals and Navy Admirals.
