There is no single deity associate with all of these concepts in Greek myth. Athena was associated with War, Eileithyia was the goddess of childbirth, and it could be argues that the Moirai Atropos was most closely associated with Death (she was one of the Fates, but the Greeks had a God of Death called Thanatos as well).

Artemis is the goddess of child birth because she helped with the birth of her brother Apollo. wich i guess would also make her the goddess of birth pain as well.
