Heads of executive departments are nominated by the President subject to the approval of Congress.
For appointed positions within the US federal government, the President and the Executive Branch nominate appointees, who must then be approved by Congress. This includes federal judges, US attorneys, and the heads of the various departments, bureaus, and agencies.

As I understand the US Military system the heads of Army, Navy, and Air Force are appointed by the Secretary of Army, Secretary of Navy and Secretary of Air Force.
The Senate has to approve the President's choices for "cabinet positions," that is, the heads of Executive branch departments. The President does have the authority to temporarily appoint a cabinet position without approval, if the Senate is not in session; that's called a "recess appointment." Recently, we had an interesting thing: the President appointed a cabinet position at a time when he said the Senate was not in session, and the Senate said they were.

The President's cabinet is a group of high-ranking officials appointed by the President that serve as advisors and heads of various federal agencies. They are also responsible for managing their departments and ensuring the efficient functioning of government programs and services.
