There is no one name for such a person, since different types of people can issue declarations, with various purposes for doing so. Often, a declaration is made on behalf of a particular government or civic organization. A good example is when a war breaks out, the legislature of that country (or the president or prime minister) will issue a Declaration of War. Another example is the American Declaration of Independence: it was written by Thomas Jefferson and several other American patriots, and announced that the American colonies no longer wanted to be ruled by England. So, those who write or issue a declaration are usually identified by their title, rather than by whether they issued the document.
