Herbert Hoover and John Kennedy donated their presidential salaries to charity. Theodore Roosevelt suposedly used his salary solely for entertaining at the White House. George Washington, initially refused a salary, but was convinced by his advisers that setting a precedent of serving without pay might later prevent a worthy person without money from running for President and so agreed to take a salary.
No President has ever refused his presidential salary. However, both Herbert Hoover and John F. Kennedy both wealthy men, donated their salaries to charity. Hoover was receiving $75,000 a year in direct pay, and Kennedy $100,000 a year.
George Washington, when he learned the US Congress had voted to pay him a sum of $25,000 a year, did not want to take a salary. However, it was pointed out to him that some future presidents might find it a real hardship to do without pay, and that he should not set the precedent of refusing a salary.
President Trump has recently indicated that he will be donating his presidential salary, which is currently set at $400,000.

George Washington was a wealthy planter (his net worth was equivalent to roughly half a billion 2010 dollars), and didn't really NEED a salary; he could have easily lived off the income from his farms. He actually told Congress that he would prefer that they simply cover his expenses as President, but for several reasons they decided it would be better to just give him a salary instead (it turns out this was brilliant: most presidents have been pretty rich, but if they had to support themselves, the position would be limited to ONLY the very rich, since they'd be the only people who could afford to take what amounts to a 4-year unpaid vacation).
Herbert Hoover wasn't as rich as Washington, but still pretty well-off (he was a mining engineer who had made a small fortune, maybe 75 million 2010 dollars). He gave at least a third of his salary to charity ... anonymously ... and used his own fortune for many things during his presidency as well. This is not quite the same thing as "not accepting pay", but it's close.
John F. Kennedy was even richer than Washington, and certainly didn't need a salary. He gave his away to charity (again, not quite the same thing, but Kennedy gave his away in its entirety, and I can't find evidence that Hoover went quite that far).
There were two that did not take their salary. George Washington and Ronald Reagan. There is alot of information about these two men and how wonderful they were.
