In the military, a rank is not 'granted', it is earned, and that process is called a 'promotion'. Promotions are earned one of two ways: by meritorious action, or by 'time & grade', whereby the promotion from PFC to LCPL is relatively automatic, assuming the enlisted man/woman has done nothing wrong and has performed his/her duties capably.

Who can grant a military rank depends upon the rank in question. For the lowest four grades of rank, it is normally that person's direct commanding officer who promotes them to the next rank. From the fifth rank on up, rank is usually decided by Congressional approval.

Rank would be Captain, Grade would be 0-3 (for the Army). An 0-3 in the Navy is a Lieutenant
