In the US there is a line of succession should a president be unable to serve due to illness or death. The next in line to serve as president is the Vice President. This was seen when President Nixon resigned and Vice president Ford became president. Should a vice president be unable to serve, then the Speaker of the House of Representatives becomes the new president.
The Chief of Staff is a position appointed by the President. If the elected President and Vice-President are unable to serve, the Speaker of the House of Representatives becomes President and selects a Chief of Staff.

The speaker of the house is very important. He/she becomes president of the US in the event the President and vice president become unable to hold their office.
If the U. S. President dies, resigns or is removed from office, the Vice President immediately becomes the President. If there is no Vice President at that time, the Speaker of the U. S. House of Representatives becomes the President. If there is no House Speaker at that time, or if the House Speaker is unable, unwilling or not qualified to be President, the President Pro Tempore of the U. S. Senate, who is by tradition the most senior member of the majority party of the Senate, becomes President. If there is no Senate President Pro Tem at that time, or if he/she is unable, unwilling or not qualified to be President, the U. S. Secretary of State becomes President. If there is no Secretary of State at that time, or if he/she is unable, unwilling or not qualified to be President, the U. S. Treasury Secretary becomes President (if qualified, willing and able). Whoever becomes President appoints a Vice President after taking office. That person takes office after both houses of Congress approve the appointment.
If the President of the United States is removed from office, the Vice President immediately becomes the new President. Then, the Speaker of the House of Representatives becomes the President if the Vice President is unable to assume the role. The President Pro Tempore of the Senate would be next in line if both the Vice President and the Speaker of the House are unable to take on the presidency.
