Article 3 maintains that sovereignty lies within the nation, that is, the nation's citizens. A monarchy is a centralized sovereignty, where only one person has authority. Since a monarch can exercise authority independently of a nation's citizens, without regard to their wishes or input, this goes against the general idea of article 3.
In the present-day United States, their New Netherlands colony was ruled by a Governor-General. The best known is the last one, Peter Stuyvesant. Although he regularly consulted with the prominent citizens of the colony, authority lay with him alone. The Netherlands themselves were a federal Republic since the late 16th century. The Estates General - a sort of national Parliament of the representatives of the Dutch provinces - was the highest authority. Since 1814 the country is a constitutional monarchy, again with Parliament as the highest power.
"We the people" is the subject of the sentence, and thus the authority that is ordaining and establishing. So basically, we did it. If by "give" you mean "write", then under some circumstances and in some locations they're allowed to write prescriptions which may legally be filled. But in general, physicians (MDs and DOs) have the broadest prescribing authority.
Authority tends to be local in nature, however, the Secretary General of the United Nations has global authority. In any given country, there will be someone who is in charge of that country and whose authority locally is greater than that of the Secretary General.
