Automatically. The first King with a particular name has, during his lifetime, no number. After that, the official title becomes, for example, 'Henry, eighth of that name, King of England... ' and so on with the full list of titles. Not even a King Chan choose his own number.

Simply by adding one to the last king of the same name. For instance, if Prince Charles were to take that name, he would be Charles III, and subsequently his son Prince William would be William V. [In fact, Charles has let it be known that he will not assume the title of Charles III].
