2. Tag can't be lost or misplaced
3. Tag can be easily found
4. Tag can be positively identified to that particular corpse
Men in the field do not always have a string to tie the dog tag to the body. In addition, in battle, while dragging dead men, uniforms have a tendency to COME OFF while dragging the body. Which is why when viewing dead bodies in Vietnam on television or book photos you will see many of the corpses appearing to be nearly naked. Their clothing (uniforms) were ripped off during first aid procedures (finding the bullet hole/shrapnel hole, then trying to patch it up) then further ripped away during the dragging process to the choppers or LZ (landing zone). Dog Tags were often tied to the jungle boot's boot lace; one tag per boot. Boots would survive alot better than flesh and cloth. So naked dead torn up pieces of flesh, if they still had a head (and mouth) then yes, place the dog tag in that hole (mouth).

The Civil War brought about many changes socially for the United States. It also brought with it changes in the way soldiers bodies were handled when they died in the field. As late as August 1862 there were Union and Confederate divisions assigned to taking the field after a battle to clear the dead. Because there was no effective ambulance corps the bodies were hauled in whatever contraption that could be built to hold bodies. Morticians had little to do with body disposal, as most of the decisions were made on the field and the bodies buried in unmarked graves. This created problems at home for Christian families that were looking for proper burials for their loved ones. Dog tags did not exist during the Civil War and most families relied on friendly soldiers who might recognize a dead soldier and write a letter informing the family of where the individual died and where they were buried. But this was not a common practice. Today, because of the problems that plagued these divisions in the Civil War, soldiers have dog tags and there is an exemplary ambulatory service to deal with the dead and injured during combat.
They did that so if they died they could be identified and the families told of their death. There was a great fear that they would be buried unknown and the civil war was before the introduction of dog tags.

US military - pretty much the way they do now. They used to have a soldiers serial number but now have social security number and they used to have a small V shaped notch along the bottom edge. The notch was thought to there in order to wedge the dogtag in-between a dead soldiers teeth. The truth is that the notch was to hold the metal tag in place on the equipment that produced them.

In older military dog tags there was a notch to place between the two upper front teeth of a soldier should he die in combat. The jaw would then be firmly wedged on the dog tag too ensure the soldier could be identified when his body was retrieved.
In case you didn't notice, soldiers have a set of tags. One with a groove, the other without. When a soldier is killed on the battle field, one of the tags is removed by another soldier to account for dead soldier. In order to keep the dead soldier from being separated from the remaining tag, the tag is placed inside the soldier's mouth with the groove placed on the lower teeth, and then the chin is kicked so that the tag is driven up into the soldier's palate.
